Emerald Care Center Midwest is seeking a PRN Physical Therapist to join our Rehab department in Midwest City, OK. The PT will assess, plan, and implement individualized therapy programs for residents in a skilled nursing setting, collaborating with a multidisciplinary team to promote functional independence and recovery.

This flexible, as-needed role supports scheduling flexibility and focuses on providing high-quality rehabilitative care with compassion.

Qualifications

• Doctoral or Master's degree in Physical Therapy from an accredited institution.
• Current state licensure as a Physical Therapist.
• Experience in a skilled nursing or long-term care setting preferred, but not required.
• Compassionate, patient-focused with strong organizational skills.

Responsibilities

• Assess patients' physical conditions, mobility, and functional abilities to develop treatment plans.
• Create and implement personalized therapy programs to improve mobility and function.
• Administer therapeutic exercises, manual therapy, and modalities.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and families on exercises, safety, and home care strategies.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Maintain accurate documentation of evaluations, plans, progress, and discharge summaries.
• Adhere to safety, infection control, and facility policies.
