Job summary

Emerald Care Center Midwest is seeking a PRN (as needed) Physical Therapist to join our Rehab team in Midwest City, OK. The role focuses on comprehensive assessment and individualized therapy to promote functional independence for skilled nursing residents.

You will work within a multidisciplinary team, delivering evidence-based interventions, documenting progress, and educating patients and families to support ongoing recovery and maintenance.

Qualifications

• Doctoral or Master's degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in skilled nursing or long-term care is preferred but not required.
• Compassionate, patient-focused and detail-oriented.

Responsibilities

• Assess patients' physical conditions, mobility, and functional abilities to develop treatment plans.
• Create and implement personalized therapy programs that address goals, improve mobility, and enhance function.
• Administer therapeutic exercises, manual therapy, and modalities to support rehabilitation.
• Regularly assess and document patient progress, adjusting plans as needed.
• Educate patients and families on exercises, safety, and home care strategies.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.

Responsibilities:

Qualifications:

• Education: Doctoral or Master's degree in Physical Therapy from an accredited institution. Licensure: Current state licensure as a Physical Therapist.
