Job summary

Evolve Therapy Services in South Range, WI is seeking a motivated Physical Therapist to join our in-house Skilled Nursing Facility program on a PRN basis.

You will assess residents, develop treatment plans, coordinate with the interdisciplinary team, and ensure CMS-compliant documentation using NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop treatment plans with short and long-term goals and implement them.
• Evaluate residents to obtain data for treatment planning and modification.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
