Job summary

Evolve Therapy Services in Greendale, WI is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility. The role is part-time or PRN, focusing on delivering high-quality resident care and evidence-based PT services.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program
• Active Physical Therapy license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide physical therapy services to residents per standards and procedures
• Adhere to documentation and billing procedures
• Develop treatment plans with short and long-term goals
• Screen residents to determine need for intervention/treatment
• Evaluate residents to obtain data for planning and implementation
• Communicate findings and recommendations to residents, families, and the team
• Complete CMS-compliant documentation in NetHealth EMR
