Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Antigo, WI. This PRN role focuses on delivering high-quality PT services to residents and collaborating with the care team.

You will screen, evaluate, and treat residents, develop treatment plans, ensure CMS-compliant documentation in NetHealth EMR, and participate in ongoing professional development to maintain licensure.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop and implement treatment plans with short- and long-term goals; communicate findings to residents, families, and interdisciplinary team.
