Job summary

Riverside Lifelong Health and Rehabilitation Smithfield is hiring a PRN Physical Therapist in Smithfield, Virginia. The PT will assess patients, develop and adjust plans of care, and work with a multidisciplinary team to maximize mobility and quality of life.

Responsibilities include evaluations, plan development, progress reassessment, patient/family education, and documentation. A Virginia PT license and CPR/BLS within 30 days are required.

Qualifications

• 1 year PT experience preferred.
• Virginia PT license required upon hire.
• CPR/BLS certification within 30 days required.

Responsibilities

• Performs evaluations and determines PT diagnoses; develops plan of care.
• Reassesses patient progress timely per guidelines and policies.
• Collaborates with a multi-disciplinary team and participates in meetings.
• Assists in training and support of team members, volunteers and students.
• Carries out treatment plans and updates care based on progress; documents progress.
