Grand River Health in Rifle, Colorado is seeking a licensed Physical Therapist for a PRN role. The PT will deliver comprehensive services across inpatient, outpatient, geriatric, pediatric, and long-term care settings, evaluating patients and creating individualized plans to restore function and reduce pain.

Qualifications

  • Current Colorado Physical Therapy License with DORA or eligibility to sit for licensing exam.
  • American Heart Association BLS certification upon hire.
  • Experience in physical therapy is preferred; knowledge of modalities, anatomy, physiology helpful.
  • Ability to assess patient needs and document care accurately.

Responsibilities

  • Evaluate patients and develop individualized treatment plans.
  • Provide therapeutic exercises, wound care, aquatic therapy, manual therapy and other modalities.
  • Collaborate with physicians, PAs, NPs, case managers, and insurance adjusters to coordinate care.
  • Supervise Physical Therapist Assistants, students, aides, and rehab staff as assigned.
  • Deliver therapy across inpatient, outpatient, geriatric, pediatric, and long-term care settings.

What You’ll Do

This role provides comprehensive physical therapy services to patients across inpatient, outpatient, geriatric, pediatric, and long-term care settings. The Physical Therapist evaluates patients, develops individualized treatment plans, and provides therapeutic interventions to restore function, improve mobility, reduce pain, and prevent disability.

  • Evaluate patients and develop individualized treatment plans based on injury, illness, surgery, or physical disability.
  • Provide a variety of therapeutic interventions, including therapeutic exercise, wound care, aquatic therapy, stretching, manual therapy, physical agents, and specialized equipment.
  • Collaborate with physicians, PAs, NPs, case managers, insurance adjusters, and other members of the care team to coordinate patient care.
  • Provide clinical direction and supervision to Physical Therapist Assistants, students, aides, technicians, and other Rehabilitation Services staff as assigned.
  • Provide therapy services across multiple settings, primarily at the Care Center, and occasionally Grand River Clinic West, and the hospital campus.
  • Performs a variety of tasks and other duties as assigned.
Who We’re Looking For

We’re looking for a licensed Physical Therapist who brings strong clinical knowledge, sound judgment, and a patient-centered approach to care. The ideal candidate is comfortable working with a diverse patient population and collaborating with patients, providers, and the broader Rehabilitation Services team.

  • Bachelor’s, master’s, or doctoral degree in Physical Therapy from an accredited program, with a current Colorado Physical Therapy License with DORA or eligibility to sit for the licensing examination.
  • Current American Heart Association BLS certification upon hire and maintained throughout employment with GRHD.
  • Previous experience in physical therapy is preferred, with knowledge of physical medicine, therapy modalities, anatomy, and physiology.
  • Ability to assess patient needs, develop and implement appropriate treatment plans, follow instructions, prioritize patient care, and maintain accurate documentation.
  • Strong communication and interpersonal skills, with the ability to communicate clearly and professionally with patients, physicians, staff, and administrative team members.
What We Offer

Our total rewards package includes

  • Pay Range: $32.35- $50.82/hour (based on experience)
  • Shift Differentials: $2–$4/hour for nights and weekends, and other incentive pay available.
  • PRN Differential: 10% differential in addition to base rate salary

Compensation may vary outside this range depending on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
