Job summary

Encore HCS's Encore at Foulk is seeking a PRN Physical Therapist to assess, develop, and deliver rehabilitation programs. You will identify needs, coordinate referrals, and educate patients and caregivers to promote independence.

Responsibilities include supervising staff, ensuring timely documentation, and collaborating with the rehab team to adapt plans of care to individual patient needs. The role emphasizes safety and high-quality service.

Qualifications

• Graduate of an accredited Physical Therapist program.
• Current Delaware state licensure.
• CPR Certification.
• Excellent verbal, written, and interpersonal communication skills.
• Proficient with therapy documentation (NetHealth).
• Must pass a medical and physical examination.

Responsibilities

• Supervise PTAs, temporary licensees, students, and support personnel as applicable.
• Deliver rehab services per plan of care with timely documentation of services.
• Attend patient care conferences and related meetings.
• Make referrals to other disciplines to address patient needs.
• Consult with rehab team to select the most appropriate regimen.
• Adapt plan of care to individual patient needs.
• Maintain a safe environment and report hazards.
• Order supplies and equipment as necessary.
• Ensure patient service quality and compassionate care.
• Evaluate patient progress and modify treatment plans as needed.
