Encore HCS at Parkside is seeking a Physical Therapist to assess patient needs, develop and deliver rehabilitation programs, and coordinate referrals to other disciplines to address comprehensive patient needs. The therapist will guide and train patients and caregivers to promote independence and functional recovery.

The position emphasizes education, individualized care plans, safety, and collaboration with the rehab team. Experience with documentation and adherence to standards is essential.

Competenze

• Graduate of an accredited Physical Therapist (PT) therapy program.
• Current IL state licensure.
• CPR Certified.
• Excellent verbal, written, and interpersonal communication skills.
• Results-oriented with strong critical thinking and problem-solving skills.
• Proficient with therapy documentation (NetHealth).
• Must pass a medical and physical examination based on job qualifications, requirements, and duties/responsibilities.

Mansioni

• Supervision of Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure and professional standards.
• Delivery of rehab services per plan of care, department, and patient schedule, with timely documentation.
• Attend and contribute to patient care, staffing conferences, and related meetings.
• M ake referrals to other disciplines and agencies to address patient needs.
• Consult with rehab team to select appropriate therapeutic regimens.
• Adapt the program and/or plan of care to individual patient needs.
• Promote a safe environment and enforce elimination of hazards.
• Order supplies and equipment as necessary.
• Maintain and report equipment maintenance and repairs.
• Put patient service first: ensure high quality care.
• Screen, examine, and evaluate patients; develop diagnosis, goals, and plan; perform treatments.
• Therapeutic techniques: exercise, functional training, manual therapy, airway clearance, integumentary repair, modalities.
• Design, fabricate, fit, and apply adaptive devices including orthotics and prosthetics.
• Select activities suited to patient capacity and interest.
• Instruct patients in mobility skills and monitor progress.
• Perform routine reexamination to adjust treatment progression.
• Perform other related duties as required.
