Job summary

Encore at Foulk in Wilmington, DE is hiring a PRN Physical Therapist to assess, develop, and deliver rehab programs that restore and enhance patient performance. You will educate patients and caregivers and coordinate referrals to other disciplines as needed.

Responsibilities include supervising assistants and staff as allowed, delivering services per plan of care, documenting thoroughly, and collaborating with the rehab team to tailor therapies.

Qualifications

• Graduate of an accredited Physical Therapist (PT) therapy program.
• Current DE state licensure.
• CPR certification.
• Excellent verbal, written, and interpersonal communication skills.
• Results-oriented with strong critical thinking and problem-solving skills.
• Proficient with therapy documentation (NetHealth).
• Must pass a medical and physical examination based on job qualifications, requirements, and duties/responsibilities.

Responsibilities

• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel per licensure requirements.
• Deliver rehab services per plan of care; adhere to schedules and document timely.
• Attend patient care and staffing conferences; contribute to related meetings.
• Make referrals to other disciplines and agencies as needed to address patient needs.
• Consult with rehab team to select appropriate therapeutic regimens for each patient.
• Adapt plan of care according to patient needs.
• Promote a safe environment and enforce safety standards.
• Order supplies and equipment as necessary.
• Maintain and report on equipment condition and needs.
• Put patient service first with high-quality care and compassionate approach.
• Screen, examine, and evaluate patients to establish diagnoses and treatment plans; design goals and methods with patient input.
• Develop and implement treatment plans including therapeutic exercise, manual therapy, airway techniques, and other modalities.
• Design, fitting, and training for adaptive devices including orthotics/prosthetics.
• Instruct patients in mobility activities and monitor progress; modify plans as needed.
• Perform routine reexaminations and other related duties as required.
