Job summary

Excelsior Care Group is seeking a compassionate Physical Therapist (PT) to join Care Connection Rehabilitation Center in Rahway, NJ. This PRN role focuses on interdisciplinary rehabilitation within a skilled nursing environment, evaluating patients, developing plans, and delivering therapeutic care.

The ideal candidate holds a DPT, NJ license, and prefers candidates with SNF experience and strong communication and assessment skills. On-site hours vary to meet patient needs.

Qualifications

• DPT from an accredited program.
• Active New Jersey Physical Therapist license.
• Minimum 2 years of experience preferred.
• Skilled nursing or nursing home experience preferred.
• Strong clinical assessment, communication, and interpersonal skills.
• Familiarity with SigmaCare and Net Health preferred.

Responsibilities

• Evaluate patients and assess physical conditions, mobility, and functional limitations.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, stretching, and hands-on treatment.
• Monitor patient progress and modify treatment plans as needed.
• Document evaluations, treatments, and patient progress accurately.
• Educate patients and families on therapy goals, home exercises, and recovery.
• Collaborate with nursing and other members of the interdisciplinary care team.
