Care Connection Rehabilitation Center in Rahway, NJ is seeking a compassionate Physical Therapist (PT) to join our interdisciplinary rehabilitation team. The role requires a Doctor of Physical Therapy (DPT), active New Jersey license, and typically 2+ years of experience. Familiarity with SigmaCare and Net Health is a plus.

Qualifications

• DPT from an accredited program.
• New Jersey Physical Therapist license.
• Minimum 2 years of experience preferred.

Responsibilities

• Evaluate patients and assess physical conditions, mobility, and functional limitations.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercises, stretching, and hands-on treatment.
• Monitor patient progress and modify treatment plans as needed.
• Document evaluations, treatments, and patient progress accurately.
• Educate patients and families on therapy goals, home exercises, and recovery.
• Collaborate with nursing and other members of the interdisciplinary care team.

Pay: $58–$65 per hour, based on experience
