Job summary

Enhance Rehabilitation in Woodstock, IL is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care, and collaborate with facility staff to provide high-quality care.

You will supervise Physical Therapist Assistants, document evaluations and progress notes, re-evaluate progress, adjust plans of care, and communicate with patients and families about outcomes and expectations.

• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
