Job summary

Enhance Rehabilitation in Illinois is seeking qualified Physical Therapists to join our dynamic team in SNF settings. As a Physical Therapist you will evaluate and treat patients according to individualized plans of care, set measurable functional goals, and collaborate with facility staff to ensure high-quality outcomes.

You will also supervise Physical Therapist Assistants, complete timely documentation, and educate patients and families about injury, progress and prognosis.

Qualifications

• Licensed PT or eligible in Illinois
• Experience in SNF or similar setting preferred
• Strong communication and documentation skills

Responsibilities

• Provide excellent care to patients and ensure high-quality outcomes
• Collaborate with facility staff to coordinate care
• Evaluate new patients and develop plans of care with measurable functional goals
• Treat patients, address concerns, and educate on progress and prognosis
• Supervise Physical Therapist Assistants
• Re-evaluate patient progress and adjust plans of care as needed
• Complete timely and accurate documentation (evaluations, re-evaluations, discharges, daily progress notes)

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
