Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in a skilled nursing facility setting. As an Enhance Rehabilitation Physical Therapist, you will evaluate and treat patients following an individualized plan of care.

You will collaborate with facility staff to deliver the highest standard of care, supervise Physical Therapist Assistants, and re-evaluate patient progress to adjust goals.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes
