Job summary

Evolve Therapy Services in Glendale, WI, is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role involves planning, directing PT services, and ensuring high-quality resident care in compliance with federal, state, and local standards.

You will evaluate residents, develop treatment plans, document care, and collaborate with the interdisciplinary team. This is a full-time/part-time/PRN position with competitive pay and benefits.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill PT services according to policy.
• Evaluate residents to plan treatment and goals.
• Develop short- and long-term treatment goals and plans.
• Communicate findings and plans to residents, families, and team.
• Screen residents to determine intervention needs.
• Complete CMS-compliant documentation in NetHealth EMR.
