The position requires licensure in the state, BLS certification, and the ability to work across post-acute, long-term care, and home health settings. This is a PRN/as-needed role with day shift hours.

Qualifications

• Appropriate education level required in accordance with state licensure.
• Must be licensed in the state(s) of practice as a Physical Therapist.
• When applicable, may require valid driver's license.
• Obtains and subsequently maintains required department specific competencies and certifications.
• Basic Life Support (BLS) for health care providers required or within the first 60 days of employment.

Responsibilities

• Plans, organizes and delivers physical therapy programs to help clients recover and improve mobility.
• Conducts examinations, evaluations, and interventions for clients with injuries or diseases.
• Documents patient information for evaluation and establishes therapeutic plans.
• Educates patients and families about treatment plans and home exercises.
• Demonstrates knowledge of current research and adheres to clinical guidelines.
• May supervise assistants, aides and students as needed.
• May focus on post-acute, long-term care and home health environments.
