Job summary

Endeavor Health is seeking PRN Physical Therapists for inpatient and outpatient care across multiple locations in Illinois, including Elmhurst and Chicago. You will evaluate, treat, and follow up with physician-referred patients, while maintaining accurate records.

Mentorship is offered for new grads and those with less than three years of experience. Positions are evergreen across sites; licensure in IL is required or eligible, with CPR certification, and possible pathways from PT I to PT II

Qualifications

• Licensed or eligible for licensure as a Physical Therapist in the State of IL.
• Graduation from a physical therapy program approved by relevant boards.
• CPR certification required.
• For PT II: minimum of 2 years experience; PT I: 0–2 years.

Responsibilities

• Provide evaluation, treatment, and follow-up services to physician-referred patients.
• Prepare and maintain accurate patient records including evaluations, progress notes, discharge summaries and billing.
