Job summary

Optum is seeking a PRN Physical Therapist for Home Health of Wilson to assess and treat patients in their homes, developing and updating the plan of care. You will evaluate functional status, mobility, pain and other conditions while coordinating with the care team.

Responsibilities include documenting visits, adjusting plans based on progress, and communicating outcomes to physicians and nurses. CPR certification is preferred; licensure in the state is required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance or access to dependable transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs and assists in plan of care.
• Provides services within the scope of practice in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family regarding plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
