Qualifications

• Bachelor's degree in Physical Therapy or related field
• Active state license
• 2+ years of experience in a skilled nursing facility environment

Responsibilities

• Screen/evaluate patients with cognitive/ADL disorders and develop care plans
• Utilize standardized assessments and evidence-based practice
• Provide rehabilitative treatments per regulatory standards
• Administer procedures as part of the plan (ADL retraining, exercises, modalities)
• Assess progress and adjust treatments in medical records
• Evaluate outcomes and modify services based on referrals
• Provide counseling to patients and families
• Communicate with supervisors and health team
• Document information in EMR and maintain patient records
• Accurately record treatment minutes
• Participate in meetings and trainings
• Supervise PT assistants and aides
• Adhere to state practice acts and company policy
