Concept Rehab, Inc. in Bryan, Ohio seeks a Physical Therapist Assistant (PTA) to join our skilled nursing team on a PRN basis. You will provide physical therapy treatments under the direction of a licensed Physical Therapist and help residents improve mobility and independence.

The ideal candidate is licensed in Ohio, graduated from an accredited PTA program, and values teamwork. SNF/geriatric experience is a plus; strong communication and documentation skills are essential.

Qualifications

• Current Ohio PTA license.
• Graduate of an accredited PTA program.
• SNF/geriatric experience is a plus.
• Dependable, compassionate, and team-oriented.
• Strong communication and clinical skills.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist.
• Assist residents with mobility, strength, balance, transfers, and functional activities.
• Follow individualized treatment plans.
• Document patient care and progress.
• Work closely with PT, nursing, and the interdisciplinary team.
• Help residents reach their highest level of independence.
