Concept Rehab, Inc. is seeking a PRN Physical Therapist to join our Battle Creek team in a skilled nursing and post-acute setting. You will evaluate patients, develop treatment plans, and provide skilled PT services to improve mobility and independence.

You will collaborate with the interdisciplinary team, document progress accurately, and support meaningful patient care on a flexible schedule. A Michigan PT license and strong clinical/communication skills are essential; SNF or post-acute

Qualifications

• Current Michigan Physical Therapist (PT) license.
• Strong clinical and communication skills.
• Skilled nursing or post-acute experience preferred.
• Dependable, compassionate, and team-oriented.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services
