Job summary

Concept Rehab, Inc. is seeking a PRN Speech-Language Pathologist (SLP) in Battle Creek, MI to join a collaborative rehabilitation team in a skilled nursing/post-acute setting.

You will evaluate speech, language, cognition, and swallowing needs; develop individualized treatment plans; provide skilled therapy; monitor progress; and complete essential clinical documentation while collaborating with nursing and the interdisciplinary team.

Qualifications

• Current Michigan SLP license.
• ASHA certification as applicable.
• Strong clinical and communication skills.
• Skilled nursing experience preferred.
• Patient-focused and dependable.

Responsibilities

• Evaluate speech, language, cognition, and swallowing needs
• Develop individualized treatment plans
• Provide skilled speech-language therapy
• Address communication, cognitive, and swallowing goals
• Monitor patient progress and modify treatment as appropriate
• Collaborate with nursing and the interdisciplinary team
