Job summary

Concept Rehab is seeking a Full-Time Physical Therapist (PT) to join our team in Hart, MI. The PT will evaluate patients, develop individualized treatment plans, and provide skilled therapy to improve strength, balance, mobility, and functional independence.

The role requires collaboration with the interdisciplinary team and supervision of PTA staff. We offer a supportive clinical environment with meaningful patient care, clinician development opportunities, and full-time benefits.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license.
• New graduates are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled physical therapy services.
• Help patients improve strength, balance, mobility, and functional independence.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with nursing, physicians, families, and the interdisciplinary team.
• Supervise and collaborate with Physical Therapist Assistants.
• Participate in care and discharge planning.
• Educate patients, families, and facility staff on therapeutic programs.
• Complete accurate and timely clinical documentation.
