Brushy Creek Post Acute is seeking a compassionate Physical Therapist (PT) for PRN coverage in Greer, SC. You will evaluate residents, develop personalized therapy plans, and collaborate with physicians, families, and the interdisciplinary team to maximize mobility and independence.

The role requires SC PT licensure, strong clinical and documentation skills, and experience in long-term care or rehab settings. Net Health and PCC familiarity is preferred.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's or Doctorate in Physical Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Proficiency with Net Health and PointClickCare (PCC) preferred.

Responsabilités

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established goals.
• Assess functional abilities, mobility, strength, balance, and endurance.
• Monitor resident progress and modify treatment plans as appropriate.
• Communicate effectively with residents, families, physicians, nursing staff, and interdisciplinary team members.
• Complete accurate and timely documentation in compliance with facility, state, federal, and payer requirements.
• Participate in care planning meetings and discharge planning activities.
• Educate residents, families, and caregivers on treatment plans, safety techniques, and home exercise programs.
• Conduct in‑service education and training for facility staff as needed.
• Recommend adaptive equipment and interventions to improve safety and independence.
• Maintain compliance with all facility policies, state practice acts, and regulatory requirements.
