Job summary

Successful candidates will evaluate residents promptly, design individualized plans, supervise PTAs, and collaborate with physicians, nursing staff, and families to optimize mobility and independence for residents.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's or Doctorate in Physical Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PointClickCare (PCC) and Casamba preferred.
• Strong clinical, communication, and documentation skills.
• Licenses & Certifications: Current SC Physical Therapist license in good standing; maintain CE requirements.

Responsibilities

• Evaluate residents within 24 hours or one business day of physician referral.
• Develop individualized treatment plans and obtain physician approval as required.
• Provide physical therapy treatments per physician orders and goals.
• Monitor progress and modify plans as appropriate.
• Supervise PTAs, therapy aides, and students in line with state regs.
• Educate residents, families, and caregivers on therapy goals and home programs.
• Complete evaluations, notes, recertifications, and other required documentation accurately.
