Job summary

The PT will evaluate and treat residents to improve mobility and independence, developing individualized treatment plans and coordinating with an interdisciplinary team.

Qualifications

• Current, valid Physical Therapist license in California.
• Graduate of an accredited Physical Therapy program.
• Experience in a skilled nursing facility or post-acute rehabilitation setting preferred.
• Strong communication, organization, and clinical documentation skills.
• Ability to work collaboratively with an interdisciplinary team.
• Compassionate, resident-centered approach to rehabilitation.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise Physical Therapy Assistants per state requirements.
• Assist nursing with Restorative Aides training.
• Communicate treatment progress and discharge planning with physicians and staff.
• Participate in care conferences and rehabilitation meetings as needed.
• Provide staff education on rehabilitation and resident care.
• Maintain timely, accurate, and compliant documentation.
