Job summary

Autumn Lake at Greenfield in Milwaukee, WI is seeking a PRN Physical Therapist (PT) to join a collaborative skilled nursing and post-acute care team.

Responsibilities include evaluating residents, creating individualized plans, delivering interventions focused on strength, balance, gait and mobility, and maintaining timely documentation. A Wisconsin PT license is required and prior SNF/post-acute experience is a plus.

Qualifications

• Active Wisconsin Physical Therapist license required.
• Strong clinical assessment and communication skills.
• Skilled nursing or post-acute care experience is a plus.

Responsibilities

• Perform physical therapy evaluations and establish individualized plans of care.
• Provide skilled interventions focused on strength, balance, gait, mobility, and functional performance.
• Monitor resident outcomes and modify treatment plans as appropriate.
• Educate residents and caregivers on mobility and safety strategies.
• Work closely with PTAs, nursing, and other rehabilitation professionals.
• Maintain timely clinical documentation.
