Job summary

Greenfield Rehabilitation Agency is seeking a PRN Physical Therapist to support our senior campuses in Pigeon Falls and Whitehall, WI. This role offers flexible PRN scheduling and the opportunity to deliver evidence-based therapy in a supportive, therapist-led organization.

You will evaluate and treat residents, develop care plans, and collaborate with an interdisciplinary team, while adhering to regulatory standards and documentation requirements. Wisconsin licensure is required.

Qualifications

• Graduated from an accredited school with a physical therapy degree (bachelor's, master's, or doctoral).
• Wisconsin Physical Therapist license as required by the state.

Responsibilities

• Perform evaluations and provide skilled physical therapy in a skilled nursing setting.
• Develop individualized care plans to improve mobility, strength, balance, and function.
• Collaborate with the interdisciplinary team to ensure continuity of care.
• Complete documentation in accordance with regulatory and company standards.
