Enhance Rehabilitation in Illinois seeks a licensed Physical Therapist to evaluate and treat patients in a skilled nursing facility, following an individualized plan of care.

You will work with facility staff, set measurable goals, document progress, and supervise Physical Therapist Assistants to ensure high quality patient care.

Formación

• Licensed Physical Therapist with current credentials.
• Experience evaluating and treating patients in SNF settings preferred.

Responsabilidades

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, educating them on their injury, progress and probable outcomes.
• Supervising Physical Therapist Assistants
• Complete timely and accurate documentation including evaluations, re-evaluations discharges and daily progress notes.

In this role, you will evaluate and treat patients following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
