Job summary

Enhance Rehabilitation in Wisconsin is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care, promoting recovery and functional independence.

In this role you will collaborate with facility staff, supervise PTAs, document progress, and educate patients and families about injury, progress and outcomes.

Qualifications

• Evaluate and treat patients according to an individualized plan of care.
• Supervising Physical Therapist Assistants.
• Complete timely and accurate documentation of evaluations, re-evaluations, discharges and daily progress notes.
