Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care, ensuring high-quality care and excellent customer service to patients and families.

Responsibilities include collaborating with facility staff, developing measurable goals, documenting evaluations and progress notes, and supervising Physical Therapist Assistants as needed to maintain optimal patient outcomes.

Qualifications

• Licensed Physical Therapist required.
• Experience evaluating new patients and developing plans of care.
• Experience documenting evaluations, re-evaluations, discharges and daily progress notes.

As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.

• Providing excellent care for our patients

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
