Job summary

Continuum Therapy Partners is seeking a Physical Therapist for Aviston Countryside Manor in Aviston, IL. A Doctor of Physical Therapy (DPT) and an active state license are required, with NPTE eligibility. One year of PT experience is preferred.

You will evaluate, develop, and implement individualized treatment plans, document patient progress, and adjust plans based on outcomes. You will educate patients and families on home programs and injury prevention. Schedule is PRN.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
