Job summary

Continuum Therapy Partners is seeking a PRN Physical Therapist to provide high-quality care in the Chicago, IL area. You’ll choose daytime shifts and join a team that maintains consistent standards and voice in care.

Requirements include a Doctor of Physical Therapy (DPT), current IL license, NPTE, and 1 year of PT experience preferred. You will evaluate patients, develop treatment plans, document progress, and educate patients and families on home programs.

Qualifications

• DPT required.
• Current, unencumbered state Physical Therapy License; must pass NPTE.
• 1 year of PT experience preferred.
• Evaluates, develops, and implements individualized treatment plans with other disciplines.
• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.
