Job summary

Glenbridge Health and Rehabilitation in Boone, NC is seeking a Physical Therapist to join our team on a PRN basis. You will evaluate residents, design and implement therapy plans, and help residents regain function and independence.

The role requires an accredited PT program graduate, an active PT license, CPR certification, and excellent communication and organizational skills. Travel to and from work as needed and willingness to work variable shifts, including weekends, are essential.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapist license.
• CPR certification required (or must obtain upon hire).
• Prior experience in a skilled nursing or long-term care setting preferred.
• Strong communication, clinical, and organizational skills.

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals
• Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility
• Monitor and document residents’ progress, response to treatment, and adjust plans as necessary
• Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care
• Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence
• Comply with all regulatory, licensing, and facility policies including documentation and infection control standards
