Job summary

Glenbridge Health and Rehabilitation is seeking a skilled Physical Therapist to join our Boone, NC team on a PRN basis. The PT will evaluate residents, design personalized plans, and provide therapeutic interventions to improve independence and function.

Responsibilities include documenting progress, collaborating with an interdisciplinary team, and educating residents and families on therapy programs. CPR certification and licensure are required, with travel as needed.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Current and valid Physical Therapist license.
• CPR certification required (or obtainable upon hire).
• Prior experience in skilled nursing or long-term care preferred.
• Strong communication, clinical, and organizational skills.

Physical Therapist Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals
• Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility
• Monitor and document residents’ progress, response to treatment, and adjust plans as necessary
• Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care
• Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence
• Comply with all regulatory, licensing, and facility policies including documentation and infection control standards

Physical Therapist Qualifications:

• Graduate of an accredited Physical Therapy program
• Current and valid Physical Therapist license
• CPR certification required (or must obtain upon hire)
• Prior experience in a skilled nursing or long-term care setting preferred
• Strong communication, clinical, and organizational skills

Additional Requirements:

• Minimum of 1 year of experience as a Physical Therapist
• Ability to travel to and from work independently as needed
• Availability to work variable shifts, including days, evenings, and weekends
