Qualifikationen

• Graduate of CAPTE-approved Physical Therapy program.
• Valid Physical Therapy license to practice in the state(s) of licensure.

Aufgaben

• Supervise Physical Therapy staff and students in accordance with licensure requirements.
• Perform screens, evaluations, progress notes, and reevaluations promptly.
• Set long- and short-term goals with residents, caregivers, and involved disciplines.
• Obtain MD orders for evaluations, treatments, recertifications, and discharges.
• Deliver and document rehab services per the plan of care.
• Participate in patient care meetings and related staff activities.
• Adjust residents' plans of care as needed.
• Promote safety for residents and staff throughout the facility.
• Prioritize residents’ needs and deliver compassionate clinical care.
• Order, clean, maintain, and report issues with supplies/equipment.
• Foster professional relationships with residents, families, and staff.
• Assist with care/discharge planning and facility training as needed.
• Travel to facilities within reasonable distance as required.
• Fulfill other duties as assigned.
