Job summary

Nationwide Healthcare Services is seeking a licensed Physical Therapist to assess, develop, and deliver rehabilitation programs in long-term care facilities. You will supervise PT staff, students, and assistants, and coordinate with physicians, caregivers, and other disciplines to maximize residents’ functional outcomes.

You will establish goals with residents and care teams, ensure orders are obtained, and maintain thorough documentation of evaluations, treatments, and progress to support

Qualifications

• Graduate of CAPTE-accredited physical therapy program.
• Valid Physical Therapy license in state(s) of licensure.
• Minimum of 3 years of experience in long-term care preferred.

Responsibilities

• Supervise PT staff, assistants, students, and ancillary staff in line with licensure and standards.
• Conduct screens, evaluations, progress notes, and reevaluations promptly upon referral.
• Set long- and short-term goals with resident, physician, caregiver, and care team.
• Obtain MD orders for evaluations, treatments, and discharges.
• Deliver and document rehab services per resident plan of care.
• Participate in care teams, staff meetings, and discharge planning.
