Job summary

Avante at Mt. Dora is seeking a PRN Physical Therapist to oversee and coordinate resident assessments, ensuring compliance with federal, state, and local regulations for our in-house therapy department.

You will provide and direct Physical Therapy services, conduct screenings, evaluate residents, implement treatment plans, and document care while maintaining confidentiality and communicating with the care team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible; 0-3 years of long-term care experience.

Responsibilities

• Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with department procedures.
• Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
• Evaluates residents to obtain data necessary for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Adheres to established confidentiality standards.
• Implements and conducts treatment as outlined in treatment plan.
• Follows management direction.
• Performs other duties as assigned.
