Avante at Mt. Dora Skilled Nursing and Rehabilitation Center is seeking a PRN Physical Therapy Assistant (PTA) to provide therapy services under the supervision of a Physical Therapist.

You will assist with assessment, planning, and implementation of treatment plans while documenting care and communicating progress to residents and families. Qualifications include an associate degree in physical therapy, successful completion of the National Certification for PTA, and current state licensure or

• Associate degree in physical therapy from an accredited program.
• Successful completion of the National Certification for P.T.A.
• Current state licensure or license eligible.
• 0–3 years of long-term care experience.

• Provide Physical Therapy services to patients under supervision of the Physical Therapist.

Qualifications:

• Ability to carry out treatment plans as designed by supervising therapist
• Ability to document daily care as per established guidelines.
• Ability to effectively communicate response to treatment intervention to residents, family, and other team members.

Education and Experience:

• Associate’s degree in physical therapy from an accredited program.
• Successful completion of the National Certification for P.T.A.
• Current state licensure or license eligible.
• 0-3 years of long-term care experience.
