Job summary

Avante at Melbourne Skilled Nursing and Rehabilitation Center is seeking a Physical Therapist to oversee and coordinate resident assessments, ensuring compliance with federal, state, and local regulations for our in-house therapy department.

The role involves providing PT services, conducting screenings and evaluations, and implementing treatment plans under physicians' orders within a collaborative team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Successful completion of National Certification Examination for Registered Physical Therapist.
• Current state licensure or license eligible.
• 0-3 years of long-term care experience.

Responsibilities

• Provides Physical Therapy services to residents and initiates referrals when appropriate.
• Conduct screenings of residents to determine need for intervention.
• Evaluates residents to obtain data for treatment planning.
• Performs specialized evaluations as indicated.
• Maintains confidentiality per standards.
• Implements treatment as outlined in the plan.
• Adheres to management direction and performs other duties as assigned.
