Stern Therapy Consultants is seeking a PRN Physical Therapist Assistant to join the rehabilitation team at Autumn Lake at Southgate in Carneys Point, NJ. The role involves delivering mobility-focused interventions and coordinating with the supervising Physical Therapist to help skilled nursing residents improve daily movement.

Responsibilities include leading therapy sessions, guiding transfers and walking, and maintaining thorough documentation.

Qualifications

• Active New Jersey Physical Therapist Assistant license.
• Ability to provide individualized treatment within an established plan of care.
• Strong communication and timely clinical documentation.

Responsabilités

• Deliver prescribed interventions that support residents' mobility goals.
• Lead therapeutic exercises and functional activities for strength, balance, and endurance.
• Guide residents through walking, bed mobility, and transfers with appropriate assistance and equipment.
• Reinforce safe movement habits and instructions for mobility devices.
• Observe responses, report changes to the PT, and adjust as needed.
• Maintain accurate treatment records and share mobility recommendations with the care team.
