Job summary

Mason Creek Transitional Care of Katy is seeking a dedicated PRN Physical Therapist Assistant to join our in-house rehab program. You will contribute to screening, evaluating, and delivering rehabilitative care for our geriatric patients within a post-acute setting.

The role requires a Physical Therapist Assistant license and is open to all experience levels; new graduates are welcome. You’ll work within our CAPLICO values and collaborate with a multidisciplinary team to support recovery,

Qualifications

• Physical Therapist Assistant license is required.
• Open to Physical Therapist Assistants at all experience levels. New Grads are welcome!

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care, creating care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Stand/walk regularly; occasional lifting, transferring, and repositioning of patients and equipment.
• Administer procedures as part of the rehabilitation plan, including exercises and modalities.
• Provide consultation and counseling to patients, families, and caregivers.
• Generate discharge summaries in compliance with regulatory and clinical requirements.
