Qualifications

• Valid State PTA license is required.
• SNF and/or Acute care experience preferred.

Responsibilities

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
