Job summary

Rehab Solutions is seeking a PRN Physical Therapist Assistant for our Home Health team in the Gainesville, FL area. This role offers flexibility and autonomy with strong clinical support and meaningful patient care.

Qualifications include an Associate’s degree and state licensure or eligibility. You will work with a collaborative team to assess and treat patients in their homes, documenting progress and educating families on care plans.

Qualifications

• Associate's degree from an accredited program.
• Valid state licensure as a Physical Therapist Assistant or license eligible.
• Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.

Responsibilities

• Assist in implementing treatment plans designed by Physical Therapists and provide direct patient care.
• Monitor and document patient progress, educate patients and families on protocols and exercises.
• Accurately document treatments and progress in compliance with policies and regulatory standards.
• Collaborate with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated care.

Requirements

• Education: Associate's degree in Physical Therapist Assistant from an accredited program.

• Treatment and Education: Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
• Documentation: Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
• Collaboration: Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
