American Senior Communities in Avalon Village is seeking a Physical Therapist Assistant (PRN) to assist under the supervision of a registered physical therapist. You will provide skilled therapy for patients across ASC Therapies & Wellness Settings, including home health, skilled nursing, and outpatient services.

Responsibilities include direct therapy per the plan of care, documenting progress, contributing to discharge plans, and collaborating with the care team to support positive outcomes

• Provide direct therapy to patients outlined in the PT plan of care
• Monitor and document patient progress
• Collaborate with the care team for discharge plans
• Contribute to case management and ensure patient-centered care
• Maintain positive interactions with patients and center staff
• Adhere to applicable state regulations concerning physical therapy
