Job summary

Avalon Village, part of American Senior Communities, is seeking a Physical Therapist Assistant to provide skilled care under a registered physical therapist in various ASC Therapy & Wellness settings, including home health and outpatient services.

You will support therapy plans, monitor patient progress, and collaborate with the team on discharge and case management, contributing to high-quality senior care in a community setting.

Qualifications

• Ability to provide skilled care under supervision of a licensed physical therapist.
• Experience assisting in ASC Therapies & Wellness Settings including home health, skilled nursing, and outpatient therapies.

Responsibilities

• Provide direct therapy to patients outlined in the OT's plan of care
• Monitor and document patient progress
• Collaborate with members for discharge plans
• Contribute to case management
• Maintain positive level of interaction with patients and center staff
• Adhere to applicable state regulations concerning occupational therapy
