Job summary

Kadima Rehabilitation & Nursing at Washington is seeking a PRN Physical Therapist to join our in-house rehab team. The PT will evaluate, plan, and administer treatments to residents, aiming to relieve pain and improve function within an established plan of care.

Candidates should have SNF/long-term care experience and hold a PA PT license in good standing. Familiarity with Net Health EDS and strong interpersonal skills are valued. All inquiries are confidential.

Qualifications

• Active Physical Therapist license in the state of PA, in good standing.
• SNF/long‑term care experience preferred.
• Familiarity with Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate, plan, and administer treatment to residents with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Ensure all treatment is delivered per established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Record and measure patient’s motor function, strength, and muscle performance.
• Monitor patient responses and modify treatment during sessions as necessary.
• Complete documentation per company policy and state/federal regulations.

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
