Kadima Rehabilitation & Nursing at Latrobe SNF/LTC is seeking a PRN Physical Therapist to join our in-house rehabilitation team. You will evaluate residents, develop treatment plans, and deliver skilled therapies to promote recovery and function.

The role emphasizes adherence to plans of care, documentation, and collaboration with families and caregivers. SNF/long-term care experience is preferred, PA license in good standing required, and a compassionate, patient-centered approach is essential.

Qualifications

• Familiarity with Net Health Electronic Documentation System.
• Ability to work with residents with varied personalities.

Responsibilities

• Evaluate, plan, and administer treatment to residents to relieve pain and improve function.

• Provide clinical support and instruct residents, families, and caregivers.
• Measure and record motor function and muscle performance.
• Monitor responses and modify treatments as needed.

Seeking a PRN Physical Therapist to join in-house rehab team at Kadima Rehabilitation & Nursing at Latrobe SNF/LTC

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
