Job summary

Kadima Health and Nursing at Luzerne, in Drums, PA, is seeking a Physical Therapist to join our team and deliver high-quality therapy in a SNF/long-term care setting. You will assess residents, develop care plans, and provide hands-on therapy to restore function and relieve pain.

Responsibilities include documenting progress in Net Health EHR, collaborating with interdisciplinary teams, and ensuring care complies with state/federal regulations.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

• Completes documentation in accordance with company policy and state/federal regulations.
